Spouse Coverage	Available in $5,000 increments. Minimum: $5,000 Maximum: 100% of employee coverage, not more than $100,000 Guaranteed coverage under age 70: $10,000 Guaranteed coverage age 70 or over: $10,000
Child Coverage	For your eligible children you may elect coverage in the amount of $2,500, or $10,000 Child benefits cannot exceed 100% of the employee’s coverage.

Voluntary Life Rates-Smoker

An open enrollment period will be available to you and your enrolled dependents each year during the calendar month prior to the policy anniversary. You and your dependents can request an increase of one benefit increment per year up to the guaranteed coverage amount without proof of good health.

Once approved for coverage over the guaranteed coverage amount you can request an increase of one benefit increment per year up to the policy maximum benefit without proof of good health.
